Dear Parent or Guardian of a NHARA alpine ski racer:
You are being invited to take part in a research study entitled, "Competitive
Alpine Ski Injury Assessment in Children" because you are a member of the New
Hampshire, New York or Vermont Alpine Racing Association. This survey should
take less than 10 minutes to complete.
Injuries related to competitive alpine ski racing have been well documented in college age athletes and much research has been dedicated to the prevention of these injuries. There is a lack of information, however, in the types and frequency of injuries in school aged pre-collegiate alpine ski racers. The purpose of this study is to identify the types of injuries that occur in pediatric, adolescent and teenage alpine ski racer with the goal of developing preventative measures to decrease the number of alpine ski injuries.
Consent to participate in this study is through the parent or guardian of the alpine ski racer if the child is less than 11 years old and through assent if your child is 11 years or older. Your participation in this survey is completely voluntary and all information is confidential. Only age and date of birth of the study participants will be recorded.
Filling out and submitting the survey gives us permission to use your data in our study. You may discontinue your participation in this study at any time by exiting the program. No information will be used unless it is submitted by you. If you would like to discontinue your participation from the study after submitting your response, please call (802)656-4259 or email jennifer.lisle@med.uvm.edu
